Description

Care Connect is a non profit organisation specialising in case management, assessment, and brokerage services. The organisation was established in 1994, specifically to prevent the premature or inappropriate admission of frail aged and younger disabled persons into residential care (ie: nursing homes and hostels). Care Connect receives funding from the Department of Human Services (Vic), the Department of Disability and Ageing (NSW) and Aged Care (Commonwealth) to provide case management services through a number of specific programmes. Care Connect also undertakes a range of contractual case management services.
