Description

A community based palliative care service providing expert and holistic services to people at home living with a progressive life threatening illness. Care also provided to patients' families and carers. Services include medical and nursing care and emotional, spiritual and practical support. Services provided to residents of the Mornington Peninsula Shire, Frankston City and City of Kingston south of Mordialloc. No charge for services except small fee charged by the Royal District Nursing Service who are contracted by PHS to provide home based palliative care nursing.
